Out-Patient Physical Therapy
Start out-patient physical therapy within 48 hours Monday-Friday. Do you feel like you are losing strength? Are you worried about falling? Are your normal activities painful? Recover in your home while benefiting from our physical therapists. Whether your condition is acute or chronic, take control of your health by taking advantage of our specialized physical therapy, including speech and occupational therapy. We will take the lead checking your insurance benefits and contacting your doctor. Sessions take place in our physical therapy gyms located in Bloomer, Chippewa Falls, Eau Claire, Osseo, and Rice Lake. Receive the best in advanced respiratory care. In July 2023, we opened a specialized, 8-bed tracheostomy care unit for short-term or long-term care. Your care team includes the RN tracheostomy care director; nursing and therapy professionals with intensive respiratory and tracheostomy training; and a certified social worker. On-Site Clinic Services
Avoid the hassle of transportation and inclement weather, as well as reduce your exposure to illness. While residing short-term or long-term at Dove Healthcare - West Eau Claire, you can elect to use our on-site physician services. We collaborated with a local medical clinic to have a physician and NP provide on-site primary care services and address concerns or medical needs that do not require a visit out to a community clinic or emergency room.
On-site visits may include exams; order writing / clarification; coordination of care; nursing direction; and referrals to heart failure clinics, anticoagulation clinics, palliative care programs, and other ancillary providers to enhance consistency of care. The cost of the visit is covered by your insurance or Medicare coverage (exactly the same as if you went to the clinic). Outcomes may include decreased need for office visits; improved response time to medical needs; and reduced admissions to the hospital.
If you elect to use this service, the physician will become your primary medical provider for the duration of your stay. Upon discharge, you will resume care with your community primary care physician. They will receive a summary of the care you received during your stay with us from our physician. Physical Therapy and Speech Therapy for Parkinson's Disease and Other Neurological Diseases
Let us help you achieve your goals! Lee Silverman Voice Treatment (LSVT) is a standardized treatment protocol administered individually in 16, one-hour sessions over one month for out-patients or in-patients. Treatments are intensive and complex; however, they are accompanied by easy to understand instructions. Treatment promotes self-awareness of increasing and maintaining vocal loudness and movements. Stroke Rehabilitation
Receive advanced care for improved outcomes. Helping you to regain independence is our top priority. While all Dove Healthcare skilled nursing facilities provide stroke rehabilitation, at Dove Healthcare - West Eau Claire you will also have access to this state-of-the-art equipment in our therapy gym:

Special End-of-Life Care Offerings

Reflections at Dove Healthcare - West Eau Claire is a distinct end-of-life care unit. Employees of Reflections receive advanced training in caring for terminally ill residents. In addition, our interdisciplinary team supports both residents and families with grief and bereavement counseling. To better accommodate your family, features include private rooms comfortably furnished to accommodate visiting guests; wireless internet; family lounge with complimentary refreshments; and a family guest suite available at no charge for extended visits.
Residents in Reflections have a physician diagnosis of a terminal, life-limiting illness with a prognosis of 12 months or less. An assessment is completed to determine eligibility for the unit. Prior to admission, we encourage you to begin completing advance directives, such as a living will, power of attorney for healthcare, and do not resuscitate (DNR) orders. Contact us to learn more.
Dove Fellows Program
This is an optional program designed to provide residents with the dignity they deserve during their last days. There are several options including, a dove figurine placed outside the door to signal to our staff it is time to say goodbye; a caregiver basket filled with helpful, comforting items; a bedside service after the resident has passed away; a treasure box to store sentimental items. No One Dies Alone
Volunteer to be a companion at Dove Healthcare - West Eau Claire, and offer support to those who are dying. From time to time, residents have neither family nor friends to be with them as they near the end of their life. Even when loved ones are able to be present, they may need to take an occasional break away. Whether it is a willingness to hold a hand, fluff a pillow, or just be present, volunteers perform a variety of comfort care measures.
To volunteer, complete the application and return it for screening and background check. Once approved, you must attend a training session.
