certified skilled nursing facility providing both short-term
rehabilitation and long-term care. The west location features Reflections,
The Suites, Dove's Nest, and both private and semi-private rooms.

nursing facility providing both short-term rehabilitation and
long-term care. The south location features Transitions Aquatic
Therapy & Exercise, the Fireside Bistro, and all private rooms.

Transitions Rehabilitation is a progressive, short-term rehabilitation
program designed to help you recover as quickly as possible and
return home successfully. In-patient and out-patient services are
provided at Dove Healthcare – South and West in Eau Claire and
Wissota Health and Regional Vent Center in Chippewa Falls.
